Out Of Network Provider

4th Trimester Physical Therapy, LLC is an out-of-network provider, which means we do not bill insurance directly. We operate on a fee-for-service basis, with payment due at the time of service. Upon request, we can provide a 'superbill' for you to submit to your insurance if you have out-of-network benefits that may cover reimbursement. We recommend contacting your insurance provider beforehand to understand your benefits.

While working with an out-of-network provider can sometimes be more costly, it comes with valuable benefits. You receive highly focused, one-on-one treatment for the entire session from a doctor of physical therapy, with a customized plan created and implemented solely by your therapist without interruptions. Most importantly, the quality of your care is not dictated by insurance limitations. You’ll also have greater communication availability between sessions, allowing for more personalized support. This dedicated and comprehensive, one-on-one approach makes a meaningful difference in both your recovery and the duration of your treatment.

Cancellation Policy :

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists' day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice of appointment cancellation will be charged a cancellation fee of 50% of the appointment cost unless they are able to reschedule pending availability of the office within the same week of missed visit. Patients that "No Show" for their appointment will be charged 100% of the scheduled appointment cost. For further details, please contact our office.
